Outreach Services Coordinator, RN - Nursing
SUMMARY
The Outreach Services Coordinator, RN coordinates all outreach services for patients at
Webster County Community Hospital (WCCH.) They are responsible for coordinating,
scheduling and assisting with services, when necessary, between patients, providers and
outreach facilities. Services include quality, risk management, endoscopy, cardiopulmonary
rehabilitation, stress tests and specialty and outpatient clinic.
This position complies with department and organization-wide policies, Critical Access Hospital
requirements, Safety, and Infection Control, and all Federal and State of Nebraska regulations
related to the performance of Webster County Community Hospital operations and requirements
of the compliance program.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Initiates assessment/reassessment based on the patient’s needs per nursing standards,
organizational policies and procedures in regards to services outlined above
Interprets, advocates and assists patient(s) according to their individual needs
Addresses patient calls effectively and courteously
Schedules and confirms patient’s procedures and/or examinations
Ensure accurate and timely processing of requests in regards to outreach services
Demonstrates ability to work both autonomously and as a team
Consistently communicates with WCCH patients and outreach facilities to ensure the best
patient experience possible
Provides support throughout procedures and/or examinations
Obtains proper consents as needed for procedures and/or examinations
Collaborates and coordinates with departmental leaders on all patient safety/risk
management issues.
Responsible for planning, administration, and monitoring of consistent readiness of all
quality management, regulatory requirements, quality improvement processes, and risk
management.
Responsible for maintaining the facility quality program; to include data collection,
aggregating and analyzing data, maintaining policies and procedures, and reporting to
Quality Committee, Senior Leadership, Medical Staff, and Board of Directors.
Coordinates the collection and reporting of quality metrics required and voluntarily reported
by WCCH, and the development of dashboards to exhibit quality metrics.
Collaborates with clinical and non-clinical teams for improvement on key performance
indicators, designs processes for new initiatives, services and other targets identified by
WCCH leadership.
Acts as internal consultant to WCCH administration and associated Medical Staff in the
areas of regulatory, process improvement, performance monitoring, and statistical analysis;
is an Ex Officio member of all quality teams.
Conducts internal audits and risk analysis as determined by the Quality Committee or
leadership and reports any safety or risk concerns to CEO.
Problem solves issues by seeking out the appropriate resources.
Synthesizes pertinent patient data to develop nursing care plans and identify nursing
priorities for care and desired outcomes.
Collaborates with the interdisciplinary team to coordinate and provide patient care
Exhibits knowledge and skill in nursing theory and ability to learn and develop competence
in patient care across the lifespan
Demonstrates ability to learn and ultimately practice in the multiple settings within the critical
access hospital
Executes therapeutic and/or supportive interventions
Evaluates the patient’s response to, and effectiveness of interventions
Evaluates patient’s progress toward established outcomes
Incorporates cultural considerations into the plan/delivery of care
Documents patient care and related information according to WCCH procedures for
documentation
Assumes responsibility in a shared or delegated assignment and supports team member’s
practices through timely and professional communication
Communicates with patient families as necessary and when needed
Serves as a role model to colleagues, students, and other care providers
Actively participates in continued learning opportunities and continued self-development
pertinent to the duties of the job
Performs other duties as assigned by Director of Nursing and Administrator

EDUCATION AND/OR EXPERIENCE
Graduation from accredited nursing program.
Experience in rural hospital is beneficial but not required.
CERTIFICATES, LICENSES, REGISTRATION
Current Registered Nurse License from the State of Nebraska
Current Basic Life Support (BLS) certification required.
Current Advanced Cardia Life Support (ACLS) required within 3 months of hire.
Current Pediatric Advanced Life Support (PALS) required within 3 months of hire.
Current Trauma Nurse Core Course (TNCC) required within 3 months of hire.
